Job Description Job Title: Primary Care Triage Clinician Reports to: Berkshire Primary Care Operations Manager Hours: Full time (37.5 hours), Monday to Friday - 8am to 4pm or 9am to 5pm Salary: Dependent on experience About Us: Berkshire Primary Care Ltd (BPC) is a GP Federation providing services for all 13 practices and 4 Primary care Networks (PCNs) in Bracknell and Ascot; Ascot PCN, Braccan PCN, Bracknell & District PCN, and The Health Triangle PCN. We serve a total population of 143,000 patients Mission Statement: Empowering General Practice to provide high quality patient focused services to encourage a healthier population. Job summary: Working in our Primary Care Hub on behalf of a group of GP Practices, within a Multi-Disciplinary Team, you will support our team of Patient Advisors who provide and signpost same day care for the local patient population. The main responsibilities of the post-holder would be: Conduct patient triage and assessments to determine appropriate treatment plans Provide primary medical care to patients of all ages, including diagnosing and treating acute and chronic illnesses Order and interpret diagnostic tests, such as blood work and imaging studies Undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home Collecting data for audit purposes; Compiling computer-generated acute and repeat prescriptions for authorisation by a Doctor Collaborate with specialists and other healthcare professionals to coordinate patient care Educate patients on preventive healthcare measures and promote healthy lifestyle choices Maintain accurate and up-to-date medical records for all patients Stay current with advancements in medical knowledge and techniques through continuing education Carry out and interpret diagnostic tests under supervision of a GP. Independently decide on and carry out treatment working under the remit of local PGDs Assess and evaluate, with patients, the effectiveness of the treatment and care provided. Refer patients for investigations and/or the appropriate specialist where appropriate Ensure the provision of continuity of care including follow-up visits. Take a holistic approach to patient care, bringing together their medical, social and psychological needs. Empower patients to take responsibility for managing their own care and treatment, where it is safe and appropriate to do so. Record the Consultation in the patient records in the EMIS Clinical System. Contribute to and write Care Plans - For those patients at risk of hospital admission, for example , in a care home or with complex needs they have a care plan which is recorded in the patients clinical record. Essential skills: Strong clinical skills with the ability to diagnose and treat a wide range of medical conditions Excellent communication skills, both verbal and written, to effectively interact with patients, their families, and other healthcare professionals Ability to work well under pressure in a fast-paced environment, particularly in critical care situations Attention to detail and strong organisational skills to manage patient records and documentation accurately Empathy and compassion for patients, demonstrating a patient-centred approach to care Ability to work independently as well as collaboratively within a healthcare team Experienced in Primary Care, would consider paramedic/nurse practitioner Registered with the appropriate Health and Care Professionals /Nursing Council Background of working in a fast paced Primary Health Care or acute environments Proven background of dealing with a diverse range of patients from all walks of life. Proven background of building positive rapport with a diverse range of internal colleagues and clients Proven ability to work autonomously whilst also having an awareness when identifying the need to access the wider clinical team or external agencies appropriately and safely First class organisational and time management skills in a fast paced environment are essential. Hold a Full UK Driving License and ideally have access to a vehicle insured for business insurance to travel between surgeries and other health care settings as appropriate Confidentiality: In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the Company as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the Company may only be divulged to authorised persons in accordance with the Company policies and procedures relating to confidentiality and the protection of personal and sensitive data Health & safety: The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Company Health & Safety policy, the Company Health & Safety manual, and the Company Infection Control policy and published procedures. This will include: Using personal security systems within the workplace according to Company guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Actively reporting of health and safety hazards and infection hazards immediately when recognised Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role Undertaking periodic infection control training (minimum annually) Reporting potential risks identified Demonstrate due regard for safeguarding and promoting the welfare of children.
