Job Title: Social Worker Grade: Band 6 Trust: Oxford Health NHS Foundation Trust Location: Whiteleaf Centre, Bierton Road, Aylesbury, Buckinghamshire, HP20 1EG (the base will be Whiteleaf Centre Aylesbury the social worker will be also required to travel to and work from Saffron House in High Wycombe Hours: Monday - Friday 9.00am 17.00pm Pay rate: £21.80 Per Hour About the Trust: Oxford Health NHS Foundation Trust provides physical, mental health and social care for people of all ages across Oxfordshire, Buckinghamshire, Swindon, Wiltshire, Bath and Northeast Somerset. The services are delivered at community bases, hospitals, clinics, and peoples homes. We focus on delivering care as close to home as possible. The post will be Countywide but based at the Whiteleaf Centre. The Social Worker will be undertaking Care Act and s117 Assessments to facilitate discharge from adult and older adult wards as well as undertaking s42 Enquiries for adults and older adults in Buckinghamshire. To be successful in applying for this role you will need: Experience of arranging discharges Social worker Qualification, SWE Experience in mental health social work, minimum 1 year In addition to experience, all Qualified roles will be required to provide evidence of: Relevant Qualifications Registration with the relevant Registration Body Role responsibilities: Embody the visions and values of Surrey & Borders Partnership Foundation NHS Trust Adhere to the code of practice/conduct of your registering body at all times where appropriate. Be accountable and responsible for own practice. Deliver, implement, and evaluate evidence-based programmes of care; set standards of care and liaise with other agencies. Participate in the multi-disciplinary assessment of individuals with substance misuse and/or mental health problems or vulnerabilities living in the community. Ensure that a risk assessment is completed for all service users Adhere to the Trust confidentiality and IG standards Be aware of the potential for vulnerability to abuse amongst the people who use this service, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies Who are NHS Professionals?
