An exciting opportunity has arisen to work with theRapid Community Discharge Service based at RBH. We are looking to recruit a band5discharge facilitator for the Rapid Community Discharge. You will be working as part of a small team who work across 7 days a week Monday – Sunday to support and enable the effective flow of patients from the acute hospital ensuring successful andtimelyonward transition to the next step in the patient’s pathway. This role entails working with multi-systems partners to ais patients’ safe discharges from the acute unit to various community settings.You will be working closely with the organisation, integrated teams, key stakeholders, and partners.


Excellent communication is a key skillrequiredfor the role; you will havepreviousexperience of working in the NHS.You will have excellent organizational and computer skills and be able to demonstrate an ability to navigate various systems and software packages.


We are looking for an excellent communicator who can work closely with key stakeholders within the Acute Trust delivering a community driven discharge to assess approach todischarge


The post holder will work as aJuniordischarge facilitator for the Rapid Community Discharge Team based at RBH

• The Junior Discharge facilitator will work autonomously to ensure a seamless interface between RBH colleaguesidentifyingpatients medically fit for discharge and then ready to leave hospital.

• Day to day you will be working within the team to ensure the effective discharges of patients –maintainingactive and up to date lists of patients ready to leavehospital

• To adopt a problem-solving approach negotiating andfacilitatingeffective discharge

• Establishing andmaintainingrelationships with other partners organizations in the surrounding areas

• Identifyany obstacles to discharge and work to resolve escalating asappropriate toprogress discharge.

• Maintain all office systems both electronic and hard copy across the service.

• To use the processes in HDT to aid swift safe discharge from the Royal Berkshire Hospital.


The postholder will need to work closely with key stakeholders within the Acute Trust – delivering a community driven, discharge to assess approach to discharge – enabling the effective flow of patients into the right service at the right time.


The team will work to ensure a seamless interface between RBH colleaguesidentifyingpatients medical fit for discharge, and then ready to leave hospital.

1. Educated at degree levelNVQ 4 or equivalent.
