Do you have a passion for supporting individuals to self manage their diabetes?

This is an exciting opportunity for an enthusiastic and motivated Band 7 Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6 month fixed term contract.

You will be joining a well-established, community based, intermediate diabetes service. The service is led by a Diabetes Consultant and consists of Diabetes Specialist Nurses, Diabetes Specialist Dietitians and dedicated administrative support.

You will be expected to deliver patient centered advice, support and care for patients with diabetes and provide advanced assessment of patients' needs and advise on appropriate diabetes management. The service delivers care in both the clinic and domiciliary setting, primarily to individuals with type 2 diabetes.

You will receive support and supervision from the existing team members including regular clinical supervision from the Diabetes Consultant.

The successful applicant must have prior experience in caring for people with diabetes. You must be self-motivating, organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to continued personal development.
• To promote the health and wellbeing of people with complex diabetes enabling them to achieve optimum glycaemia control through informed self management.
• To empower and support people with diabetes to self-manage their condition, guiding them towards a ‘normal’ lifestyle and reducing morbidity and premature mortality by detecting and preventing complications
• To provide education and support to both people with diabetes and those who care for them.
• To promote strong, effective and regular collaborative working between all health care professionals and stakeholders along the diabetes care pathway
• To provide a link between the GP and consultant led specialist teams.

Epsom and St Helier University Hospitals NHS Trust provides a range of medical services to approximately 490,000 people living across southwest London and northeast Surrey.
