We are recruiting for Mental Health Practitioners who wishes to join our Crisis Resolution Home Treatment Team. You should share our passion and drive to ensure that our patients receive quality and compassionate care.

The Mental Health & Learning Disability Services on the Isle of Wight have been transforming services over recent years to ensure we provide services that will meet the needs of the island population. We have a strategy to provide a ‘No Wrong Door Service’ whereby minimising the need for people who come to the MHLD service attention to repeat information and be seen by numerous services.

In order to ensure Acute Mental Health services support people through the No Wrong Door strategy, we have been transforming the current crisis pathway to a single Crisis Resolution Home Treatment Team which involves expanding the scope and capabilities of the current Home Treatment Team.

The CRHT team is seen as a vital mental health service dedicated to delivering high-quality care and support to individuals experiencing mental health crisis. We are looking for people who share our vision of our CRHT service playing a pivotal role in providing timely and effective interventions, ensuring that individuals receive the care they need in the least restrictive setting possible.

As a member of our Crisis Resolution and Home Treatment Team, you will be at the forefront of providing crisis intervention and support to individuals and their support networks in the community who are experiencing acute mental health crises. Your role will be dynamic and rewarding, involving collaboration with a multidisciplinary team of mental health professionals. Across Hampshire and the Isle of Wight healthcare services are evolving and transforming to meet the continuing care needs of our population. Now is a very exciting time to join Isle of Wight NHS Trust as we enter this period of change.

This year Isle of Wight NHS Trust and Portsmouth Hospitals University NHS Trust have strengthened our partnership and formed a group giving us an opportunity to work together to be innovative in improving patient care and outcomes as well as improving the experience and opportunities for our people across the two organisations.

We have also been working with other local NHS Trusts across our region to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust - Hampshire and Isle of Wight Healthcare NHS Foundation Trust - to be formed by April 2024.

People employed in our community, mental health and learning disability services by the Isle of Wight NHS Trust on 30th April 2024, will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) on 1 May 2024.

The Isle of Wight is a unique and stunning gem located off the South Coast of England, a short ferry journey from Portsmouth and Southampton, with journey times taking from as little as 10 minutes. The island offers an amazing coastline with beautiful countryside, year-round events, festivals and award-winning attractions.

Key Responsibilities:

Conduct comprehensive collaborative assessments of individuals in crisis, including risk assessments.

Develop and implement crisis care plans tailored to each individual's needs.

Provide crisis intervention and support to individuals in their home environment or other community settings.

Collaborate closely with psychiatrists, nurses, social workers, and other healthcare professionals to ensure coordinated care.

Offer psychological support and psychoeducation to individuals and their families.

Monitor and evaluate the effectiveness of crisis interventions, adjusting plans as needed.

Assist in de-escalating crisis.

Promote recovery-oriented care and the least restrictive treatment options.

Maintain accurate and timely documentation of all interactions and interventions.

Qualifications:

We are pleased to receive applications from Registered Mental Health Nurses (RMN), Registered Social Workers with Mental Health experience.

Previous experience in crisis intervention, mental health assessment, or related roles is preferred.

Strong interpersonal and communication skills.

Ability to work effectively in a fast-paced and dynamic environment.

Empathy, compassion, and a non-judgmental approach to mental health.

Commitment to promoting recovery and person-centred care.

Flexibility to work rotating shifts, including evenings, weekends, nights and Bank holiday
