The post will offer you a range of clinical experience in adult diabetes. The post is based in the Diabetes centre so provides excellent multi disciplinary team working experience with experienced diabetes nurses and podiatrists. The post includes supporting adults with all types of diabetes (including antenatal and gestational diabetes) in hospital, outpatient and community settings.
There will be opportunities to deliver diabetes structured education for both Type 1 and Type 2 diabetes, and to train other healthcare professionals. The post holder will be trained as DAFNE and DESMOND educator if you do not already hold the accreditation. You will have the ongoing support of an experienced Diabetes Dietitian and excellent admin support.
Applicants must have had Dietetic experience in the NHS at under graduate or post graduate level - interviews will be held in person/face-to-face.
Interested candidates are advised to look to the job description and person specification found alongside this advert for a full and detailed description of duties, roles and responsibilities associated with this post. For the right applicant we would consider a Band 5 to Band 6 development post and to be considered for this you will need to have completed post registration experience in the NHS.
The diabetes team within the Nutrition and Dietetics Service provides dietetic support to adults with diabetes. We are committed to providing a comprehensive, compassionate and patient centred service to our patients. Structured education programmes are available to all people with diabetes and these are co-delivered by Diabetes Specialist Dietitians, Diabetes Specialist Nurses and Podiatry team.
We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services. They are part of the NHS – yet decisions about what they do and how they do it are driven by independent boards. Boards listen to their Council of Governors and respond to the needs of their members – patients, staff and the local community.
Foundation trusts provide what the health service wants, yet are also free to invest quickly in the changes to the local community needs, in striving to be the best, and in putting their patients first.
Please refer to the job description and person specification attached for further information regarding the main responsibilities of the role
