Job Duties:

• Provide high-quality nursing care to individuals within the community setting.
• Conduct comprehensive assessments and develop care plans based on individual needs.
• Administer medications, perform wound care, and assist with various treatments as required.
• Collaborate with healthcare professionals and multidisciplinary teams to ensure coordinated care delivery.
• Educate patients and their families on health management and self-care techniques.
• Monitor patient progress, evaluate outcomes, and make necessary adjustments to care plans.
• Maintain accurate and up-to-date documentation of patient interactions and interventions.

Requirements of the Community Nurse:

• RGN qualified/NMC registered with valid live pin
• Recent UK nursing experience
• Must have Community/District nursing experience
• Proven experience as a community nurse or in a similar role.
• Strong clinical skills and knowledge of community healthcare practices.
• Excellent communication and interpersonal abilities.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Strong organizational and time management skills.
