Summary

Solutions4Health are excited to be initiating the Healthy Worcestershire programme on behalf of Worcestershire County Council. This new, exciting initiative aims to empower the local community to improve their general health and wellbeing, as well as supporting local individuals with help and information around community safety issues like doorstep crime, scams, and fraud. We are looking to recruit an Healthy Worcestershire Wellbeing Coach. This is a full-time job with a salary of £25,000 to £27,000,depending on experience. The Wellbeing Coaches will be responsible for delivering a combination of OTAGO exercise sessions designed to improve strength, balance and flexibility, along with running workshops on community safety; covering topics such as scam awareness and fraud prevention. The successful candidate will also be expected to provide brief tailored advice to support individuals to make general positive changes with regards to health and wellbeing. Main Duties & Responsibilities Delivery of exercise sessions designed to improve strength, balance, and flexibility across the locality for participants aged 18+. Deliver awareness raising sessions on a range of topics to include community safety, health, and nutrition. Using your relevant training and experience to enhance the sessions and empower participants to make positive and sustainable behaviour changes. Triage all referrals into the service and book them onto relevant sessions. Undertake general health assessments using our Integrated Health System (IHS) to assess and support individuals to make better lifestyle changes. Refer participants to other local services where required, i.e., local stop smoking services. Work closely and develop sustainable relationships with local partners including Pharmacies, GPs and secondary care providers in the development, promotion, and delivery of the Worcestershire Health project. Assess and interpret clinical information regarding referred participants whilst maintaining confidentiality and data protection. Ensure the programme is reflective of individual needs of the participants and are mindful of health and safety standards. Interact with referred participants, recording details, monitoring adherence. Ensure that all sessions and activities are monitored in line with agreed targets. Maintain and regularly update participant information by providing data both electronically and in paper format to contribute to the timely monitoring and evaluation process. Establish good rapport and relationships with participants to maintain participation and involvement in the programme. Communicate effectively and interact collaboratively with participants, partners, and stakeholders to resolve issues or problems arising. Have a flexible approach to working hours to meet the service's needs. Maintain accurate records and information systems in line with company policies. Locate, evaluate, and deliver sessions from appropriate venues allowing access to participants around Worcestershire. Promote and develop greater independence, quality of life and a healthy active lifestyle among older people referred into the service. Gather information on all referrals made to the service and follow up contacts accordingly. Strive for the continual improvement of the service, using client feedback and experiences to drive quality. Motivate, support, and reassure participants, whilst being considerate of and understanding barriers to change. Maintain accurate records and information systems in line with Solutions 4 Health policies. Ability to travel across Worcestershire.
