Summary


An opportunity has arisen to join this exciting team; you will be part of a team and have responsibility for a varied caseload of Community Nursing clients, the main focus being hospital avoidance and expectation of hospital discharge. The main patient scope is IV antibiotics and DVT diagnosis. The team cares for Patients in their own home or in a clinical setting this care closer to home model includes flexibility to respond rapidly for unplanned care to avoid Emergency Department attendance. You will work under the direction of senior nurses within the team. You will need to work flexibly to meet the demands of the service and be able to work effectively with partnership agencies and the Acute Trust over 7 days a week 08:30 to 21:30 on a rota basis.

Under the direction of the Band 6 Clinical Intervention nursing team you must assess, plan, implement and evaluate patient care demonstrating evidence based/best practice, considering own clinical confidence and competence. You must implement enhanced programmes of care.

To work with primary and secondary services to provide high standards of clinical intervention and to provide nursing care for service users accessing the range of Clinical Intervention services

To implement enhanced clinical care programmes that are of the highest quality & standards and in line with Walsall Healthcare NHS Trusts policies and procedures

To work within a multi-agency/multi-disciplinary team adopting a ‘no blame’ culture.

To adhere to Walsall Healthcare NHS Trusts policies, procedures and protocols to promote the quality of patient experience, ensuring the highest standard.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.

Undertake assessment of care needs, development, implementation and evaluation of programmes of care, within Scope of Professional Practice.

Demonstrate high level of judgemental/analytical skills when interpreting and performing patient assessments and care planning.

Maintain accurate and up to date nursing records, maintaining patient confidentiality, and statistical records as required by the service.

Negotiate and agree with patient, carers and other care professionals, individual roles and responsibilities with regard to actions to be taken and outcomes achieved, referring on to other services or professionals as appropriate.
