Summary


This Fellowship is aimed at senior trainees who are in possession of the FRCA (or equivalent) or CCT holders seeking specialised training in high risk anaesthesia for vascular surgery. However, post-primary candidates displaying aptitude and interest will be considered. The Fellow will gain experience in the pre-operative assessment and optimisation of high risk vascular patients and gain proficiency in the conduct and interpretation of CPET. They will manage patients for surgery under regional and general anaesthesia, for open and endovascular, elective and emergency procedures.

The Fellow will take part in the trainee 1:8 on call rota, being on call for Obstetrics, Intensive Care Unit or Theatres as required by the department. The trainees are supported by an anaesthetic consultant and an intensive care consultant on call from home. The post is for 6 -12 months commencing August 2024.

Clinical Experience

These goals will be dependent on the grade and prior experience of the successful applicant:
• Appreciation of the implications of major comorbidities on the perioperative management of vascular patients.
• Use of national guidelines, research, risk stratification, clinical assessment and CPET in assisting with pre-optimisation, perioperative management, and decision-making.
• To have a comprehensive understanding of the benefits and risks of regional, combined regional/general and general anaesthetic techniques for major vascular surgery.
• Independent preoperative assessment (in-patients and clinic setting) of patients having major vascular surgery and formulation of a perioperative care plans including any necessary pre-optimisation.
• Contribution to multidisciplinary team meetings in planning for patient care.
• Independent performance and interpretation of CPET (for patients undergoing vascular surgery and major colorectal resection) and understand the implications of the results on surgical choices.
• Counselling of patients regarding their risks and choices.
• Planning for blood conservation strategies and blood loss management.
• Use of cardiac output monitoring inpatient management.
• Independent management of patients undergoing peripheral vascular surgery, abdominal aortic aneurysm repair (open and endovascular) and carotid endarterectomy in the elective and emergency setting under either general or regional anaesthesia.

Clinical Experience

These goals will be dependent on the grade and prior experience of the successful applicant:
• Appreciation of the implications of endovascular techniques on anaesthetic management including, but not limited to, anaesthesia in an isolated site, renal protection and contrast nephropathy, management of patients with significant comorbidities and surgery performed under regional anaesthesia.
• To gain exposure to a range of endovascular operations including TEVAR and REVAR.

Teaching and Presentations
• Participation in regional trainee teaching of vascular anaesthesia.
• Presentation of an update lecture to the anaesthetic department.



Clinical Audit and Research

The Fellow is expected to take part in clinical audit of topics pertinent to vascular anaesthesia and CPET.

Timetable

The exact timetable will be tailored to fit the Fellow’s specific interests. The Fellow will take part in the trainee on call rota, being on call for the intensive care unit, obstetrics or theatres dependent on the requirements of the department.
