Description
We are seeking a highly motivated and compassionate Wellbeing Assessment Social Worker to join our team. As a Wellbeing Assessment SW, you will play a crucial role in assessing the mental, emotional, and social well-being of individuals in need. You will work closely with clients, families, and other healthcare professionals to develop and implement personalized care plans that promote overall well-being.
The ideal candidate will have a strong background in social work and a deep understanding of mental health issues. They must be able to demonstrate empathy, excellent communication skills, and the ability to work effectively within a multidisciplinary team. Responsibilities
Conduct comprehensive assessments of individuals' mental, emotional, and social well-being
Collaborate with clients and their families to develop and implement personalized care plans
Provide counseling and support to individuals in need, helping them cope with life challenges and improve their overall well-being
Collaborate with other healthcare professionals, such as psychologists and psychiatrists, to provide comprehensive care
Advocate for clients and assist them in accessing appropriate resources and support services
Maintain accurate and detailed records of client interactions and progress
Stay up-to-date with the latest research and best practices in social work and mental health assessment
Requirements
Bachelor's degree in Social Work or a related field
Current state licensure or certification as a Social Worker
Proven experience in conducting assessments and providing counseling services
Deep understanding of mental health disorders, trauma-informed care, and social determinants of health
Strong interpersonal and communication skills
Ability to work effectively in a team environment
Excellent documentation and record-keeping skills
Compassionate and empathetic demeanor
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £27.71
