Summary


We are looking for anAdministrator to join our Community Nursing TriageTeam, you will support the Community Nursing servicetodeliver care to people in their own homes by assisting in the triage of new referrals as well as providing administrative support to the team. We offer a supportive environment with opportunities for further career development.
• To be the central point of contact alongside a trained clinician for all new referrals into the district nursing service, taking messages, obtaining information for new referrals, advising patients about visits, referring as appropriate and answering routine enquiries. 
• You will assist in the triage and allocation of new referrals, advise patients about visits, refer as appropriate and answer routine enquiries. 
• Type confidential correspondence, reports and other documentation to a high standard within agreed timescales. 
• Maintain effective communication across the service and deal in a professional, helpful, and sensitive manner with patients, carers, staff and other agencies by telephone and face to face. 
• Assist in the efficient and accurate recording of data on all required electronic systems including the Trust system RIO. 
• Duties will include collating and inputting data, ordering stock supplies, general administrator tasks and patient contact.
