Summary
An exciting career opportunity has arisen for a Band 6 Clinical Triage Nurse to join the Wokingham Community Nursing Team – this role is both office and practice-based.
The Clinical Triage Nurse will provide clinical support, assistance and professional expertise to all callers to the Clinical Nursing Triage, including both healthcare professionals and patients within Wokingham. The service is the main point of access for Wokingham Community Nursing taking, referrals from professionals and patients requiring healthcare services and the Trust Health Hub.
Within this position you will be required to demonstrate sound clinical reasoning and have a positive enthusiastic approach to the role. This role is partially office based and also working with patients in their own homes as part of the urgent nurse support. Also, the role may be supporting one of our community clinics on site at Wokingham.
We are a friendly and supportive team, looking for highly motivated RNs who demonstrate a passion for delivering care to the community. Your role will be pivotal in ensuring the local community receive the highest quality of care in their own homes.
As a Community Triage Nurse, no day is the same and you will be communicating with patients and service providers of varied needs.
Please note you must currently be in the UK with access to a car for work purposes to be eligible for this role.
Main duties of the job
• To work as a key member of the Triage Team
• To offer sound expert clinical advice that oversees entry into our service.
• To maintain up-to-date Statutory and Mandatory training and be prepared to rotate into clinical services as necessary to maintain clinical skills.
• To hold professional registration and adhere to professional codes of practice and conduct.
• To work in conjunction with administrative support staff and the community nursing service to provide high quality, clinical triage using knowledge, skills, critical thinking and professional judgement.
• To assist the administrative support staff where required by accepting the transfer of cases requiring additional clinical advice, reviewing recorded clinical information and offering advice.
• To provide urgent clinical support to patient in their own homes and in-house clinic support.
• To control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
• The Community Triage Nurse will possess the ability to prioritise tasks, cases and situations appropriately to ensure effective delivery of services during periods of high demand.
• To liaise and communicate clinical information verbally and/or via telephone to Clinical Nursing Staff as appropriate to ensure continuity of care for the caller.
