An excellent opportunity has arisen for an experienced Occupational Therapist to work in the Waltham Forest Integrated Discharge Hub . The team is a friendly MDT team consisting of Physios, nurses , OT, admin , working with Bridging carers and external partners to facilitate discharges.
The integrated discharge hub is designed to ensure patients receive enhanced discharge planning and a seamless transition beyond the acute hospital environment
The Integrated Discharge Hub provides a central route to refer and access social care and community based services to support and facilitate patient discharges. The hub works closely with the other NELFT community services and local authority partners. The hub uses the discharge to assess and Home First principles to facilitate assessments of patients to manage and meet their ongoing needs.
The successful candidate will also receive a welcome payment of 10% of agreed basic salary (exclusive of HCAS) , this would be payable as 50% at end of the probationary period and 50% after 2 years working in the role.
Be part of a rota leading discharge hub meetings to facilitate timely discharges .
To provide clinical expertise through triaging referrals and facilitate appropriate discharge destination, equipment and care needs for patients in acute hospitals.
Work closely with acute , social care and community services to expedite discharge times, minimising barriers and improving patient experience across all community discharges (Pathway 1 – 3) within the system.
To be responsive to patients whose needs may change immediately following discharge by providing urgent OT assessments/ interventions and arranging follow up from community teams . This can include complex moving and handling risk assessments and joint working with carers to ensure patients are safely cared for.
Supervision of junior staff and deputising for the Operational lead (when required).
COVID-19 Vaccination

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement – London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).y
