Summary


The post holder will provide a professional, proactive, and sensitive service to drug monitoring patients and their relatives. Will act as point of contact for patients, carers and the wider healthcare team to provide (in conjunction with the neurology specialist nurses) high quality patient care through proactive identification of needs which require a level of clinical knowledge utilising approved tools and procedures.

To work with Neurology Specialist Nurses and the wider healthcare team acting as a resource to ensure the provision of appropriate services, to maximise the health and quality of life of the patient, improving the quality and efficiency of healthcare delivery. The post holder will be responsible for the coordination of prescriptions, administrative function within the neurology pathways arranging investigations, appointments, and Multi-Disciplinary Team discussion in a timely manner. To ensure targets are not breached and patient’s needs are met. Will be expected to manage and prioritise the running of the service and work flexibly to the needs of the service, which can often be demanding and unexpected.

Act as a point of contact for patients, carers and wider health care teams. Triaging incoming calls and initiate appropriate responses according to protocols and individual pathways.

Deal with situations in a sympathetic, calm and supportive manner.

Escalating issues to the nurse specialist or other healthcare professionals where appropriate.

Proactively identify situations to organise diagnostic tests and treatments.

Take effective actions with the relevant teams and departments to ensure a seamless pathway.

Manage own caseload, making pre planned outbound telephone calls to patients, plan care and monitor effectiveness of services and patients’ self management, triaging as appropriate.

Maintain accurate and concise records of patient contacts to ensure seamless communication pathways for the wider healthcare team.

Act as patient advocate and facilitator to resolve issues.

Collect patient information, aimed at the provision of quality care with an involvement with the pharmacy homecare process.

Managing the day-to-day administrative function of the service prioritising workload and reviewing work practices in accordance with service development

Maintain and organise the drug monitoring email account, delegating to a specific nurse when needed or appropriate

Assist the clinical nurse specialists to develop education and information.

Utilise software (Excel) to enter, document and monitor all aspects of patient care coordination and service delivery
