Our Community Therapy Team would be the perfect career choice for you! You will be given the opportunity to broaden your clinical experience whilst being well supported to manage a varied caseload. Based in Coventry, you’ll be joining us in a cosmopolitan, culturally diverse city in the centre of England, easily accessible to all major cities.

Our Community Therapy Team play a vital role in supporting people in their home to live well, achieve their goals, and promote self-management through understanding causes of health decline to offer meaningful rehabilitation. Identifying support required, though clinical reasoning and creating person centred goals are key to the role. You will work closely with our Physiotherapists, Dietician, Assistant Practitioners, Rehab Assistants, Care Navigators, whilst linking to work collaboratively with our District Nursing teams.

We have designed our developmental posts to ensure you get the right support to develop your clinical skills and are involved in exciting transformational projects impacting on improving patient care. The opportunities to invest in your professional experience and development are extensive. This post would suit current Band 5 looking to widen their experience, knowledge base and skill set. On successful completion of your Occupational Therapy competencies within a set timeframe you will automatically be promoted to a band 6 Occupational Therapy role.

Coventry & Warwickshire Partnership Trust provides a full range of expanding physical, mental health and learning disability services to children, young adults, adults and older adults over many sites across Coventry, Warwickshire and Solihull.

Services are provided to a population of over one million people living in Coventry and Warwickshire and also a wider geographical area in some of our specialist services. We see on average around 5,000 patients every day.

You will be assessing patients in their own homes, in temporary or permanent residential or nursing care to maximise and empower rehabilitation potential, return to baseline, supporting self-management and adjustment to living life differently.

You will be able to provide timely assessment of the patient’s needs, set appropriate patient-centred goals and put plans in place for their ongoing management. Caseload management is supported by delegation to dedicated Assistant Practitioners and Technical Instructors. You will be integral to support the development of our unregistered workforce.

As part of Out of Hospital Services therapy work collaboratively with our Advanced Nurse Practitioners, District & Specialist Nurses, and local General Practitioner’s. The service works alongside Occupational Therapists in Adult Social Care. You will experience the benefit of having close working relationships with Therapy teams at UHCW and with access to CWPT Social Prescribing, Speech and Language Therapy and Community Psychology, we are able to work together to enable positive patient outcomes, and promoting the value that you bring to patient care and your job satisfaction.
