Crynodeb
Plan and organise own caseload and clinical workload appropriately on a day-to-day basis, taking into account specific issues such as patient health needs, social and emotional circumstances, appropriate information sharing etc. Plan and deliver quality individual nursing interventions according to child/young persons needs, and work in partnership with the child/young person and their family to ensure they benefit from a good experience within the community setting. Work collaboratively as part of an integrated team (including other specialities and teams) around the child/young person and ensure effective undertaking of additional clinical care tasks, offering support, advice, training and guidance to unqualified practitioners undertaking clinical tasks. Attend case review meetings and participate in beneficial debates and discussions with regards to individual patient care to enable other members of the team to benefit from and share information, knowledge and experience. Be involved with patients admission\discharge from hospital ensuring provision of appropriate level of service within the community.
