Summary

Radis Community Care have an opportunity for a Full Time Care Planner to schedule visiting care calls within the Pontypridd area.

About the role

As a Planner you will be required to schedule and allocate work to a team of Visiting Care Workers on a daily / weekly basis. You will be forward thinking in creating capacity for new clients and have the ability to maximise our rostering software to do so.

Building strong relationships with existing clients, local authorities, health agencies, service users, care staff and management, including staff at other organisations, to best meet service user needs and maximise potential business opportunities and our private funded care customers.

Participate in the out-of-hours ‘on-call’, which is 1 in every 3 weeekends.

    Skills and Qualifications required for the role

      • 1 years experience in Health & Social Care
      • The successful candidate will have an NVQ Level 2 in Health and Social Care with a desire to work towards Level 3.
      • You will have the ability to set and maintain high standards of quality and professionalism in all areas of work.
      • You will have good written and verbal communication skills with the ability to engage with staff, service users and outside agencies in a professional manner.
      • This role requires you to undergo an enhanced DBS check.
      • A driving licence with access to a vehicle will also be required for the role.
