An exciting opportunity has arisen for a permanent, 36hours per week, band 5 registered nurse on our GP led community ward Redwood Suite. Redwood is a 12 bedded ward with single en suite rooms and is based within the Rhymney Integrated Health and Social care resource centre in the town of Rhymney, South Wales.
We are looking for experienced, motivated and committed RNs to join our team. Due to the isolated nature of the ward and the requirements in an out of hours situation for the RN to lead the ward in the absence of the ward manager, experience of working as an RN within a similar ward environment will be required.
The ward follows a graduated care model that focuses on step up or step down care. This provides support to patients in the community who require step up care to a community hospital and also stepping down patients care from acute settings for ongoing rehabilitation and assessments. The ward has support from the community resource team therapists which includes twice weekly physiotherapy visits and support from an occupational therapist, rehab assistant and social worker where required. Palliative care support is provided by our colleagues from St David’s Hospice.
The shift pattern is 12 hours and internal rotation between days and nights will be required.
To assist with the assessment, planning, delivery & evaluation of patient care.
To provide prescribed nursing care without direct supervision.
To contribute to the development/evaluation of nursing practice on the ward.
To act in accordance with the NMC Professional Code of Conduct and other policies.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
