Intermediate Care Services are well established within Walsall with a fully integrated domiciliary service and excellent relationships with Social Services and the Acute Sector.
An opportunity has arisen to develop your role as a Community , Urgent Community Response (UCR) ACP, meeting the demand of the New Ambition for Older People –Standard 7 high level of performance indicators. This role will also develop your skills in working in our palliative care Centre, contributing to the ACP weekend rota.
You will be part of a Multidisciplinary/Multi Agency Team and have responsibility for a varied urgent caseload of clients in residential and nursing homes as well as in their own homes, working under the direction of the Service manager. You will be assisting to develop Older People’s Services to develop and support this client group to remain in their homes and to receive support with enhanced services to avoid hospital admissions.

· Use advanced skills and in depth expert knowledge to assess the physical and psycho – social needs of a frail elderly client group with complex multiple long term conditions, residing in local residential, nursing home as well as the trust’s stroke rehabilitation unit, instigating therapeutic treatments, including extended and supplementary prescribing, based on best available evidence in order to improve health outcomes.
· To provide advanced in-depth knowledge of frail elderly patients with long term conditions to ensure best practice across WHT exploring and implementing ways of preventing unscheduled admission for patients into secondary care.
· To make and receive direct referrals to and from nurses, consultants, general practitioners and other members of the multi-disciplinary team e.g. podiatrists, physiotherapists and occupational therapists.
. To facilitate an expert level of advanced care to patients with managed care needs, acting as an accountable role model through the demonstration of evidence based practice.
. To work with and provide clinical support and highly specialist advice to associated nursing staff across the integrated health and social care teams.
. To set up the community ward model and its associated systems which include working in partnership with medics, nursing and allied health care professionals, information technology providers and performance teams.
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
The post holder will be responsible for the leadership and management of a UCR Team, supporting elderly care throughout the Walsall Healthcare NHS Trust whilst also working closely with other outside agencies - including social services, Residential, Nursing homes and voluntary organisations.
The post holder will work to strengthen clinical and professional leadership and inspire colleagues within the multi-disciplinary team to improve and maintain evidence based best practice whilst developing their own professional practice.
The post holder will be responsible for delivery of advanced level of care to adult patients who are elderly and/or frail with long term conditions and at risk of hospital admission.
