Summary
The post holder will within a multiagency team to identify early and engage to help at risk families delivering evidenced based episodes of care. The post holder will have a shared responsibility for managing skill mix teams The post holder will participate in practice development and public health initiatives The post holder will have responsibility for maintaining own professional accountability through supervision and updating.
