IntroductionAcorn by Synergie is recruiting for a Senior Support Worker to join a team in Newport. This role involves supporting individuals in their own homes by delivering high-quality, person-centred care while also providing leadership and guidance to a team of support workers.
The successful candidate will help ensure service users receive safe, dignified and personalised care that promotes independence and wellbeing, while supporting the supervision of staff and maintaining care standards.
Key Duties:- Provide person-centred care to service users in their own homes, promoting independence, dignity and wellbeing.
- Support individuals with daily living activities in line with their care plans.
- Encourage and facilitate social interaction and participation in community-based activities where appropriate.
- Monitor service users' health and wellbeing and report any changes or concerns to the Care Manager or relevant professionals.
- Assist with medication administration and ensure accurate documentation of medication records.
- Supervise, mentor and support a team of support workers to maintain high standards of care.
- Conduct one-to-one meetings and supervision sessions with staff.
- Assist with the induction and training of new team members.
- Provide feedback to the Registered Manager regarding team performance and development needs.
- Maintain accurate and up-to-date records of care provided, incidents and service user progress.
- Ensure documentation meets the requirements of the Regulation and Inspection of Social Care (Wales) Act 2016 and Care Inspectorate Wales standards.
- Support the development and implementation of care plans and risk assessments.
- Ensure care practices comply with safeguarding procedures, health and safety requirements and the Social Services and Well-being (Wales) Act 2014.
- Assist in maintaining appropriate staffing levels and coordinate with the Registered Manager to cover shifts or absences.
- Ensure care delivery is tailored to individual service user needs in line with Welsh care standards and regulations.
- Participate in service audits, quality assurance checks and action plans aimed at improving care delivery.
- Level 3 Diploma in Health and Social Care (or equivalent).
- Previous experience working in a support worker or senior care role, preferably within domiciliary care.
- Willingness to register with Social Care Wales.
- Experience administering medication and working with complex care plans.
- Good understanding of Welsh social care regulations, including Care Inspectorate Wales requirements.
- Strong understanding of person-centred care and supporting individuals with complex care needs.
- Strong leadership skills with the ability to motivate and guide a team.
- Excellent communication skills, both written and verbal.
- Strong organisational skills with the ability to maintain accurate records and documentation.
- £15.42 per hour.
- Part-time hours of 24 per week.
- Participation in an on-call rota including evenings and weekends.
- Permanent opportunity.
