Summary
Clinical Provision of specialist, high quality, expert nursing care and support for patients and their carers with a diagnosis of breast cancer, being assessed, coordinated or cared for in liaison with the multi-disciplinary team. Work within the colorectal Surgical Team providing specialist, high quality, expert nursing care and collaboratively with multi-disciplinary team to ensure best practice and optimal outcomes for patients. Creates and utilises opportunities to promote health and well-being of patients including advice for patients with a strong family history, genetic probability of colorectal cancer. To perform an effective key role within the multi-disciplinary team to deliver cancer waiting time targets and pathway communication utilising the 2 -week wait colorectal triage service utilizing both face- to- face or telephone ensuring this encompasses the needs of patients in line with standards of good practice and cancer validation review recommendations. To undertake all nursing procedures in accordance with Trust policies and ensure that these procedures and policies are understood and adhered to by all staff. Act as role model, clinical expert and advocate for service users within the Trust and externally. To act as a specialist resource for patients and the clinical team by providing comprehensive advice, information, teaching and training. Increasing productivity and efficiencies within the unit by providing nurse-led services and providing early intervention to prevent unplanned admissions. To communicate with patients and significant others with empathy and reassurance regarding assessment, care and treatment. Ensure high-quality care through evidence-based, personalized treatment, supported by a comprehensive assessment of needs, thoughtful care planning, and continuous evaluation of all interventions. Acts as a key driver for service improvement/ development. Managerial Responsible for effective management of financial resources within the team. Participate with the management of risks and incident management and complaints within the service. Provide leadership and effective management to staff employed within the service. Initiate projects to raise the profile of the service and team members. Active involvement in the development of the service locally and nationally. Conduct appraisals for designated staff within the team in line with the trust appraisal process. To ensure staffs compliance with statutory and mandatory training. Sickness management within the nursing team To actively promote the use of Friends and Family cards to gain patient feedback and develop any necessary action plan. To ensure the electronic roster is an accurate reflection of the hours staff have worked and completed within the correct time frames Education and Training Update your own clinical and specialist knowledge and participate in clinical audits. Provides educational support for healthcare providers and healthcare consumers. Develop and maintain links with professional networks. Research and Audit Ensure clinical effectiveness by appraising current research and applying to practice where relevant. Develops and participates in relevant nursing and medical research. Involvement in team clinical effectiveness, particularly those involving patient pathways. Coordinates the development, implementation and review of nursing policies, standard operating procedures relating to colorectal services ensuring evidence based.
