Summary
Working with our Community Nursing team, you’ll provide high-quality care to people in their own homes, helping them stay safe and independent. Your role will make a real difference to patients and families by supporting their health and wellbeing.
You’ll be part of a supportive team that values collaboration and professional development. We’ll help you build your skills while delivering care that reflects compassion, dignity and respect.
What you’ll do:
• Carry out holistic assessments and plan individualised care
• Deliver nursing interventions such as wound care, medication administration and health education
• Communicate effectively with patients, families and partner agencies
• Maintain accurate records in line with professional standards
• Contribute to service improvements and uphold CHCP policies
What we’re looking for:
• Registered Nurse (or working towards final assessment)
• Understanding of community nursing and evidence-based practice
• Ability to work independently and as part of a team
• Basic IT skills and willingness to learn
• Flexible approach to meet service needs, including occasional weekends and bank holidays
