Crynodeb
The post holder will collaborate with the Optimizing Care Lead and the broader Integrated Community Nursing team to provide high-quality care while fostering self-management among patients within the existing caseload. Responsibilities include assessing and determining the appropriate level of support required for current patients, utilizing motivational, coaching, and negotiation skills to promote independent management of healthcare needs and facilitate timely discharge from the service.
Additionally, the post holder will support and educate family members and carers to enhance patient self-care and overall management. It would therefore be a requirement that the successful candidate will hold existing clinical band 5 skills. Due to the nature of the post the applicant will be expected to travel across many sites within the Hull & East Riding area providing care in the community, therefore would need to demonstrate that they could do this safely and effectively using their own vehicle and must have a UK driving licence with business use insurance and the vehicle must be suitable for business purposes. Whilst a base will be identified, staff can be asked to work anywhere within Hull & East Riding.
