Summary
We are excited to offer an opportunity to join the dynamic and forward-thinking Frailty Virtual Ward team in Newham University Hospital. This innovative service is at the forefront of delivering integrated, patient-centred care to some of our most vulnerable residents, supporting older adults with frailty to receive safe and effective treatment in the comfort of their own homes.
As part of the Frailty Virtual Ward, you will be working within a multidisciplinary team that includes GPs, geriatricians, community nurses, therapists, pharmacists, and social care colleagues. Together, we deliver proactive and responsive care to prevent avoidable hospital admissions, support early discharges, and improve quality of life for our patients.
This is a unique and rewarding role for a compassionate, motivated, and collaborative healthcare professional who is passionate about innovation in frailty care and committed to driving forward integrated working across acute, community, and primary care services.
• Act as the first point of contact for the Frailty Virtual Ward team.
• Coordinate and manage virtual ward referrals, admissions, and discharges.
• Schedule and organise daily MDT meetings, maintaining accurate records and action logs.
• Update and maintain patient tracking systems and electronic health records.
• Liaise with internal and external services to support timely patient care.
• Provide general administrative support, including data entry, reporting, and correspondence.
