Summary

To provide comprehensive care coordination, secretarial, and administrative support to the clinical team, ensuring high standards of patient care, effective communication, and accurate clinical record keeping. The role acts as a key point of contact between patients, clinicians, and external healthcare services, supporting patients with long-term conditions and complex needs. Key Responsibilities Act as a first point of contact for patients, clinicians, and external services regarding care coordination and clinical administration. Support the coordination of care for patients with long-term conditions and complex needs, working closely with the multidisciplinary team (MDT). Manage clinical correspondence including referrals, hospital letters, test results, reports, and onward actions. Provide comprehensive secretarial support to clinicians, including document preparation, workflow management, and task prioritisation. Maintain accurate, timely, and high-quality patient records in line with practice policies and information governance requirements. Liaise effectively with community services, hospitals, social care, and other healthcare providers to support continuity of care. Support MDT meetings, care planning processes, and follow-up actions as required. Ensure confidentiality, data protection, and information governance standards are upheld at all times. Use clinical systems (e.g. EMIS, Docman) to manage workflows and maintain data quality. Contribute to service improvement and support the smooth day-to-day running of the practice. Skills & Experience Essential Excellent organisational and time-management skills with the ability to prioritise workload effectively. Strong written and verbal communication skills. Ability to work independently as well as collaboratively within a team. Good IT skills with a high level of accuracy and attention to detail. Professional, reliable, and patient-focused approach to work. Desirable Previous experience working in General Practice or a healthcare setting. Knowledge and experience of clinical systems such as EMIS and Docman. Understanding of care coordination, long-term conditions, or NHS pathways. Experience of working within a multidisciplinary team.
