Summary
Key Responsibilities: Implement the CKD alert bundle at both Aintree and Royal Liverpool sites. Develop and deliver a CKD care bundle for patients admitted with CKD and unplanned hospital admissions. Contribute to CKD-related quality improvement work across the Integrated Care Board (ICB), including developing an integrated proactive care approach. Collaborate with multidisciplinary teams to embed improvements in practice Gain hands-on experience with QI methodology and its practical application. Collect, analyse, and publish data/findings from the improvement work to share learning locally and nationally.
