Summary
Job Purpose To provide clinical services in the Heart Transplant and Mechanical Circulatory Support Programme as well as the general Cardiology/ Heart Failure, Respiratory Medicine service at Harefield Hospital. Clinical Responsibilities To ensure good communication is maintained within the Transplant service at Harefield Hospital, working in collaboration with medical and surgical colleagues, other departments and services. To take history, examine and record all findings of all patients admitted under the Transplant Service, including those in the Intensive Care Unit. To assist in Out-patient sessions when required. To arrange for the transfer of patients to/from other hospitals or discharge home; including a transfer/discharge summary. To perform minor surgical procedures such as chest aspiration, chest drain insertion, central venous lines and arterial blood gases etc. Under the supervision of a Specialist Register if necessary. To dictate / input all urgent discharge summaries only and ensure that these are forwarded to the patients General Practitioner or hospital of transfer as soon as possible after the patient is discharged. Supervise and train junior staff allocated to the Transplant Service. To work as a full member of the multi-disciplinary team, including participation and presentation at weekly Multi-Disciplinary Team Meetings and other meetings in the Division and Trust. To undertake regular ward rounds to ensure effective management during the in-patient stay. To keep the Ward Sister, Specialist Registrar and Consultant informed of any patient who is giving cause for concern. To communicate to patients relative the progress of that patent. Participate in the Medical Audit Programme and update relevant patient database programmes. Participate fully in the service and training programme of the Transplant Service and Heart Division. To accept that occasional emergencies and unforeseen circumstances may occur, and to respond to the needs of the service when they arise. Professional Responsibilities To seek and maintain professional qualifications. To maintain current knowledge of cardiology / heart failure, respiratory medicine and transplantation, and related areas appropriate to carrying out these duties, and to participate in a recognised programme of continuing medical education and professional development. To set and maintain high professional standards.
