ASW - Social Worker ( 2+ years post qualification experience) - Independence & Access
Description
We are seeking a dedicated and compassionate ASW - Social Worker with a minimum of 2 years post-qualification experience to join our Independence & service team. In this role, you will play a vital part in supporting individuals to maintain their independence and improve their quality of life. You will be responsible for assessing the needs of clients, developing and implementing care plans, and providing direct support. Your expertise will also include crisis intervention, advocacy, and connecting clients to relevant resources within the community. As part of an interdisciplinary team, you will collaborate with other professionals to ensure a holistic approach to care. The ideal candidate will have a strong commitment to social justice, an understanding of the challenges faced by diverse populations, and the ability to build trusting relationships with clients. You will be involved in case management processes and have the opportunity to contribute to policy development. Responsibilities
Conduct comprehensive assessments to determine clients' needs and eligibility for services.
Develop, implement, and monitor individualized care plans in collaboration with clients and their families.
Provide direct support and guidance to clients, helping them to navigate challenges and access resources.
Advocate for clients’ rights and access to services, ensuring they receive appropriate support in the community.
Coordinate with healthcare providers, community organizations, and other stakeholders to facilitate necessary services and interventions.
Maintain accurate and up-to-date client records in accordance with organizational policies and regulatory requirements.
Provide crisis intervention and support to clients facing urgent challenges or emergencies.
Encourage and support clients in developing life skills and achieving personal goals for independence.
Requirements
Bachelor's or Master's degree in Social Work from an accredited institution.
Minimum of 2 years post-qualification experience in a social work setting, preferably within independence services.
Strong knowledge of social welfare legislation and community resources.
Excellent interpersonal and communication skills, with the ability to build rapport with clients from diverse backgrounds.
Demonstrated experience in conducting assessments and developing care plans.
Crisis intervention experience and the ability to handle high-stress situations calmly and effectively.
Proficient in record-keeping, report writing, and utilizing electronic case management systems.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £23.73
