Summary
Conduct thorough assessments of client needs and situations to determine appropriate assistance.
Develop and implement individualized service and care plans, monitoring and evaluating client progress to modify plans accordingly.
Provide counseling, support, and crisis intervention to clients facing various challenges.
Advocate for clients to secure necessary services and resources, such as housing, food assistance, and healthcare.
Coordinate and collaborate with multi-disciplinary teams, healthcare providers, schools, government agencies, and other community resources.
Maintain accurate, confidential, and up-to-date documentation and client records, ensuring compliance with federal, state, and local legal requirements.
Facilitate support groups and community outreach programs to promote social change and community well-being.
