Summary


Rotherham Care Group is committed to re-shaping services to ensure that more services are community based whilst also offering assurances that any funded aftercare services meet the need of our service users.

The post holder will lead a new service that has specific focus on supporting service users who are in receipt of care and treatment in a hospital provision, under the Mental Health Act, or receiving community care packages commissioned and delivered within Section 117 aftercare arrangements.

The post holder will need to lead and coordinate key professionals across health services, Local Authorities and the Independent Sector to ensure that effective commissioning of service provisions occurs and that once stabilised, is monitoring and reviewed to deliver measurable goals and outcomes for the service user.

-Ensure oversight over all Rotherham patients who are eligible for S117 aftercare

-Ensure routine reviews are completed in line with partnership agencies

-Offer assurance that any funded aftercare packages are meeting the service user’s identified needs

-Provide training to RDASH and RMBC colleagues around S117 aftercare, the process and expectations

-Liaise with patients, families and carers where necessary

-Implement host commissioner guidance in respect of out of area placements

- Monitor and quality check commissioned packages of care to ensure they remain effective

-Lead and coordinate key professionals across RDASH, local authority and the Independent Sector to review and monitor commissioned placements.

-Provide clinical expertise to key members internally to the organisation

-Participate in working groups to develop services which impact beyond immediate area of work
