Summary
Job Purpose As a Clinical Pharmacist, you will work as part of a multidisciplinary team in a patient-facing role. You will take clinical responsibility for areas of chronic disease management and conduct structured medication reviews for patients with complex polypharmacy. You will also provide expert advice on medicines use and safety, supporting GPs and practice staff with prescribing, medicines reconciliation, and the implementation of national and local prescribing guidance. This is a pivotal role within the practice, improving patient outcomes, reducing workload pressure on GPs, and supporting high-quality, safe prescribing. You will act as an independent prescriber and contribute to key practice initiatives including medicines optimisation, QOF, and enhanced services. This description is not exhaustive and may be subject to change to meet the evolving needs of the surgery. Key Responsibilities: Patient-Facing Clinical Work 1. Conduct clinical medication reviews for patients with long-term conditions and/or polypharmacy. 2. Implement prescribing changes and arrange appropriate monitoring. 3. Lead multi-morbidity and medicines optimisation clinics. 4. Deprescribe medicines where appropriate and support patients to manage their medicines safely. 5. Provide follow-up and monitor treatment outcomes. 6. Support the management of common, minor or self-limiting conditions within scope of competence. 7. Offer structured advice and support to patients with medicine-related queries. Medicines Reconciliation and Hospital Discharge 1. Reconcile medications following hospital discharge or care transfers. 2. Identify and resolve discrepancies without needing GP intervention. 3. Liaise with hospital pharmacy teams as required. Prescribing Support (Non-Patient-Facing) 1. Manage the repeat prescribing reauthorisation process. 2. Review medications reaching review dates and prescribe as appropriate. 3. Ensure relevant monitoring is arranged for patients on high-risk medicines. 4. Support the management of weekly prescriptions/nomad boxes. 5. Promote safe, cost-effective prescribing in line with local and national guidelines. 6. Implement changes from formulary updates or safety alerts. Medicines Optimisation and Quality Improvement 1. Contribute to QOF, enhanced services and other practice-led clinical initiatives. 2. Monitor prescribing trends and contribute to audits and action plans. 3. Work proactively to reduce medicine-related harm and improve outcomes. 4. Support the implementation of prescribing protocols and procedures. Communication and Collaboration 1. Work closely with GPs, nurses, community pharmacy teams and practice staff. 2. Attend and contribute to clinical and prescribing meetings (e.g. PLTs, ICB events). 3. Provide training and support to other members of the team on medicines-related matters. 4. Recognise and act on safeguarding concerns, making referrals in line with policy. 5. Opportunities to supervise and train within the MDT including a pharmacy technician trainee.
