Summary

1. Patient Care and Clinical Management Diagnose, treat, and manage a wide range of acute and chronic medical conditions. Conduct consultations through face-to-face, telephone, or online appointments. Provide preventive care, health education, and lifestyle advice. Prescribe medications safely and appropriately, following national and local guidelines. Deliver holistic care that considers physical, psychological, and social factors. Arrange investigations, interpret results, and follow up appropriately. Refer patients to secondary care or community services when specialist input is required. Support the practice in meeting QOF, IIF, and defined service targets. Work collaboratively with the wider primary care team, PCN colleagues, and community services. Provide clinical supervision for medical students Participate in the practices duty doctor rota and home visit schedule (as agreed). Engage in continuous professional development and appraisals in line with GMC requirements. 2. Continuity and Coordination of Care Maintain ongoing relationships with patients and families, often over many years. Coordinate multidisciplinary care by working closely with nurses, pharmacists, allied health professionals, and social services. Support patients in navigating the wider healthcare system, ensuring seamless care transitions. 3. Preventive and Population Health Participate in screening, immunisation, and health promotion programmes. Identify and manage risk factors for disease within the practice population. Contribute to local and national public health initiatives. 4. Quality and Governance Engage in clinical audit, quality improvement projects, and appraisal processes. Adhere to GMC standards and clinical governance frameworks. Ensure accurate and timely clinical documentation using electronic health records. Participate in safeguarding, infection control, and medicines management. 5. Education, Training, and Leadership Support the education of medical students, GP trainees, and other clinical staff (if applicable). Take part in continuing professional development (CPD) and annual revalidation. Contribute to service development, clinical leadership, and innovation within the practice or Primary Care Network (PCN).
