Social Work Team Service Manager - EIPH and Hospital
Job description
37 hours per week

In Bridgend, the Early Intervention and Prevention Hub acts as the front door to our integrated services for all Adults. The Hospital Social Work Team fulfil the critical link between our local hospitals and our community services to help people get home as soon as they are able. Together, the service fulfils a crucial function in our health and social care system. We're often the first point of contact and aim to work with people and their families to identify their strengths and resources and ensure they get the help they need. So, it's important we get this right!

This role will give you a unique opportunity to build on the success of our existing teams and create a model that delivers long term outcomes for the people of Bridgend. If you enjoy a challenge and are confident working across teams and organisations this may be the role for you.
