Summary
Support the nurses within the specialist care home support team to identify residents likely to be in their last year of life and offer proactive care planning through MDT meetings, advice and joint visiting Liaise with hospital specialists where needed to establish appropriate treatment plans for care home residents Work closely / liaise with General Practitioners to share care planning Participate in and/or lead reflective practice sessions with the team and care home staff
