Summary
To assist the Primary Care Network team to deliver improvements to the services we provide to Mid Dorset Patients To support the participation of national programmes for long-term conditions Ensure robust and supportive referrals in place for patients where services dictate. Form productive and supportive relationships with practice staff in support of long-term conditions where possible and relevant. Contribute to the agenda by collaborative working with varying levels of communication. Support audit/referrals and case reviews. Where guided, develop safety netting advice where patients are referred for long-term conditions. Support the PCN in its delivery of peer-to-peer learning events. Work alongside other organisations, delivering long-term condition agenda. Be the nominated contact for respiratory, Mental Health, Cancer Maintain accurate records and documentation. Ensure compliance with data protection and confidentiality regulations. Generate reports and summaries when requested or to support initiatives. Provide educational material to patients when required by email, post, text message. Assist in the organisation of events and meetings, minute taking and report writing. Maintain robust and responsive lines of communication. Respond to queries and demands of the role quickly and efficiently.
