Summary

This is an exciting opportunity to join the Reablement service. The role will be based at either Wythenshawe Hospital. The successful candidates will join a service that prides itself in seeing people achieve their goals and return to independence.

The post holders will work within a hospital setting, they will co work with health and social care professionals, to identify and refer people to the reablement service. The post holder will work in a person centred, strength based way to achieve positive outcomes for the individual whilst helping them to maintain their independence, they will provide and promote choice and control to the individual which will help them transition from hospital back to their local community.

The post holder will be required to manage a caseload, undertake social care assessments and reports to ensure effective statutory service support.

The post holder will be required to recognise and identify safeguarding, taking a person centred approach to safeguarding in accordance with the Care Act 2014. You will need to be able to identify and manage risk and where required provide an effective response in line with local policies and procedures.

The post holder will have an holistic approach to work, with a keen focus on reducing inequalities and supporting citizens and their families to develop their resilience and improve their outcomes along the Reablement pathway.
