Summary

This post is offered on a part time (6 PA) basis although other less than full time applications will be considered. The post will complement our existing consultant workforce, and individuals will work cross-site but the successful applicant will work predominantly supporting Hospital at Home. The Community Services Division is supported by the Divisional Clinical Director, Allied Health Professionals, Community Nursing and Therapists. The post being offered to complement the existing workforce. Clinical responsibilities will include: Patient Care providing comprehensive medical care to adults within the community, including diagnosis, treatment, and management of acute and chronic conditions. Hospital at Home expansion clinically leading the Hospital at Home programme and its expansion developing effective in reach services that facilitate admission alternatives, shortest possible length of stay and support patients in receiving care in the place they call home through step up and step-down care. Clinical assessment conduct in depth assessments and consultations, generating comprehensive management plans for the current episode and future crisis / escalation. Collaboration Work collaboratively with allied professionals, primary care networks, emergency department staff, inpatient and rehabilitation, divisions of medicine and acute care, radiology teams (in hospital and CDCs), adult social care, VCSE and other stakeholders to ensure coordinated and seamless care for patients. Promoting care at home where appropriate. Prevention and Care Plans Collaborate with patients and multidisciplinary teams to develop comprehensive care plans, including Respect plans that honour patients' care preferences and values. Strong focus on preventive measures to enhance overall health outcomes and reduce the risk of hospital admissions. Health Promotion Engage in community health initiatives and promote preventive care strategies that empower patients and their advocates to take charge of their health. Working to develop neighbourhood models of care to ensure they are effectively integrated into the Trusts ways of working. Patient Education Educate patients and families about health conditions, treatment options, and lifestyle changes to empower them in their health journey. Supporting them to self-manage their conditions to promote the most effective use of community resources. Documentation Maintain accurate and up-to-date patient records, adhering to NHS standards and regulations. Quality Improvement Participate in clinical audits and quality improvement initiatives to enhance service delivery and patient care. Supervision and Training Provide guidance and support to junior staff and medical students, fostering a culture of learning and development. Provide supervision and development support to Advanced Care Practitioners and allied care professionals within the trust and (when agreed) educational support within community partners eg care homes. Working arrangements will consist of the following: Hours across Monday to Friday currently 0830:1700. Supporting Professional Activities in accordance with the Trust Framework, with at least a minimum of 1 SPAs included in the core 6PA timetable. Additional SPA or DCC time may be commissioned in line with the skills of the successful candidate and changing requirements of the service.
