Summary
Work as part of a multi-disciplinary team providing community based holistic assessments Maintain a high standard of patient focused care reflecting an inter-disciplinary approach to rehabilitation and self-management and help provide excellent post discharge care for patients recovering from respiratory exacerbations Develop good communication links with patients and carers as well as consultants and service staff Establish and promote joint working with other services across professional and organisational boundaries.
