Summary
We are looking for a passionate, motivated, Social Worker to join our Hospital Social Work Team. We support and work with Adult’s over 18 to facilitate a safe transfer of care from hospital and then continue to work with them in the community to maximise their independence and assess long-term care needs. You must enjoy working in a fast-paced environment and working closely with our multi-disciplinary system partners.
Your main responsibilities as a Hospital Social Worker:
You will manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, and being part of a duty rota.
You will deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible.
The team is looking to appoint an individual who is enthusiastic to engage in adopting strengths based, person centred assessments.
You will engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks.
You will be required to provide an effective response to safeguarding enquiries and support for people with impaired capacity to make decisions.
Essential requirements as a Hospital Social Worker include:
Degree in Social Work, or relevant Social Work qualifications recognised by the Central Council for Education & Training in Social Work/Registration with SWE
• Ability to develop and sustain effective working relationships within service users, carers and groups
• Excellent communication and engagement skills with a commitment to improving service delivery.
What can I expect?
This is a full-Time post of 37 hours per week Monday-Friday and includes a Saturday 1 in 6 which is reflected in your pay and a subsequent day off.
