Summary
Job Title: Care Coordinator – Domiciliary Care
Location: High Wycombe
Reporting to: Registered Manager
Salary Range: £27,000-£29,000 per annum
Contract Type: Full Time – 37.5 hours per week
Job Purpose
As a Care Coordinator with a focus on domiciliary care, you will play a key role in supporting the coordination and delivery of high-quality home care services. Working closely with the administrative and management team, you will help ensure Service Users receive consistent, person-centred support that promotes independence, dignity, and safety in their own homes. This role is ideal for someone with hands-on domiciliary care experience who is looking to develop in care management and coordination.
Key Responsibilities
Care Coordination Support
Assist in conducting care needs and risk assessments for new and existing Service Users.
Help develop, implement, and review personalised care and support plans that reflect Service Users’ goals and preferences.
Ensure plans are outcome-focused and tailored to individual needs, promoting independence and wellbeing.
Scheduling and Service Delivery
Support the creation and maintenance of staff rotas to ensure efficient service delivery.
Work with the team to allocate suitable carers based on skills, continuity, and Service User compatibility.
Respond promptly to changes in care needs or staff availability, updating schedules as needed and be willing to step in and cover when needed.
Communication and Liaison
Serve as a point of contact for carers, Service Users, and families, addressing routine queries and concerns.
Liaise with healthcare professionals, social workers, and the internal team as directed by the Service Manager.
Attend care reviews and team meetings both internal and external, contributing to continuous improvement in service delivery.
Monitoring and Quality Assurance
Conduct spot checks, supervision visits, and service user reviews to ensure high standards of care.
Record observations, report concerns, and escalate safeguarding issues promptly.
Assist with audits and ensure care documentation is completed and maintained in line with regulations.
Administrative Support
Maintain accurate records on care management systems, including visit logs, incident reports, and communication notes.
Support with data entry, rota management, and compliance tracking as required.
Help ensure all processes align with CQC standards, GDPR, and internal policies.
Essential Skills and Competencies
Experience working in domiciliary (home-based) care.
High level of understanding person-centred care and the needs of individuals living independently.
Strong organisational and time-management skills.
Clear and compassionate communication style.
Ability to work under pressure and adapt to changing priorities.
Full UK driving licence and access to a vehicle
Qualifications and Experience
Level 2 or 3 qualification in Health and Social Care (or working towards).
Previous experience as a senior carer, field care supervisor, or support worker in domiciliary care.
Familiarity with care planning software and digital record-keeping.
Knowledge of safeguarding, the Care Act 2014, and care regulations.
Desirable
Experience in supporting individuals with learning disabilities, mental health conditions, or complex care needs.
Understanding of local authority frameworks, direct payments, and care funding.
