Summary
Main Duties and Responsibilities:Clinical: Provide autonomous clinical assessment, diagnosis, and treatment for patients with undifferentiated and undiagnosed conditions. Undertake minor illness clinics and acute care sessions. Respond to urgent, same-day appointment requests, including triage and prioritisation. Carry out home visits, including assessments of frail or housebound patients. Support chronic disease management (e.g., asthma, COPD, hypertension) under supervision or as part of a team. Administer and prescribe medications within your scope of practice (if qualified). Manage clinical risk and refer patients appropriately to secondary care, community services, or GPs. Participate in emergency or urgent medical response within the practice as required. Team Working: Work as part of a multidisciplinary team including GPs, nurses, healthcare assistants, pharmacists, and social prescribers. Participate in clinical meetings and contribute to ongoing service improvement. Support practice with quality improvement initiatives, audits, and evidence-based practice. Mentor and support less experienced colleagues or students where appropriate. Administrative: Maintain high standards of clinical documentation and coding in patient records. Use clinical systems effectively (Emis) Assist with care planning, discharge follow-ups, and coordination of care.
