Care Closer to Home The CCN team provide a clinical service, the aim of the service is to provide a skilled nursing resource for children, young people and their families, reducing hospital admissions where possible and facilitating early discharge. The team accept a number of referrals from hospitals and community agencies. Paediatric OutpatientThe CCN team is also responsible for managing and staffing all paediatric outpatient clinics across ABUHB. Specialist SchoolsThere are four special needs schools within Gwent supported by the CCN team
