Responsibilities
Conduct comprehensive assessments of clients' needs and situations to determine eligibility for services.
Develop, implement, and evaluate individualized care plans with clients, ensuring they are tailored to meet specific needs and goals.
Provide counseling and emotional support to clients, assisting them in coping with challenges and making informed decisions about their care.
Act as an advocate for clients, helping them access resources, benefits, and services that enhance their quality of life.
Monitor and review client progress regularly, making adjustments to care plans as necessary to ensure continued effectiveness.
Collaborate with other professionals, including healthcare providers and community organizations, to coordinate care and services for clients.
Maintain accurate and up-to-date case records, documentation, and reports in compliance with organizational and legal requirements.
Requirements
Master's degree in Social Work from an accredited program.
Current and valid state licensure as a Clinical Social Worker (LCSW) or equivalent.
Proven experience working in adult social services or a related field with a focus on case management.
Strong understanding of social work principles, ethical standards, and legislation affecting adult care services.
Excellent communication, interpersonal, and problem-solving skills to effectively engage with clients and stakeholders.
Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
Demonstrated commitment to promoting diversity, equity, and inclusion in practice and service delivery.
