Referral Officer (SO1)
Description
We are seeking a dedicated and proactive Referral Officer (SO1) to join our dynamic team. This pivotal role is essential in facilitating the referral processes within our organization, acting as a key point of contact between clients and various services. The ideal candidate will possess strong communication skills and an understanding of the referral protocols, ensuring that clients receive timely and appropriate assistance when needed. As a Referral Officer, you will be responsible for assessing individual client needs, coordinating with multiple departments, and maintaining accurate records of all referrals made. Your ability to build rapport with clients and stakeholders will be instrumental in effectively linking clients to the resources they require. A successful Referral Officer will also demonstrate a commitment to enhancing client experiences through efficient service delivery and will contribute to continuous improvement initiatives. Responsibilities
Assess client needs through interviews and assessments to determine appropriate referrals.
Coordinate with internal departments and external agencies to facilitate client access to necessary services.
Maintain and update accurate records of all referrals and client interactions.
Monitor and evaluate the effectiveness of referral processes and client outcomes.
Prepare reports on referral statistics and trends for internal analysis.
Develop and implement referral strategies in alignment with organizational goals.
Provide support and guidance to clients throughout the referral process.
Requirements
Bachelor's degree in social work, human services, or a related field.
Proven experience in a client-facing role or in case management.
Strong interpersonal and communication skills.
Ability to work collaboratively with diverse client populations and service providers.
Knowledge of local resources and referral networks.
Excellent organizational and time management skills.
Proficient in using computer software for data entry and record-keeping.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £20.59
