Summary
Total Social are working with Hackney Council who are looking for an experienced locum Adult's Social Worker for their Hospital Discharge Team.
The job will involve the usual frontline tasks, including:
* Completing Care Act Assessments
* Conducting Care Act Reviews
* Performing Safeguarding and Human Rights Assessments
* Completing CHC Checklists
* Collaborating with hospitals, families, and other agencies
