Summary
To work with the prison service partners in providing appropriate support and risk assessments for patients who are subjected to prison service risk management procedures such as ACCT and segregation procedures. Maintain accurate patient and health care medication records on the electronic patient record and provide information as appropriate within the secondary mental health team. To enter information on inphase system relating to the safety and security of records. Support patients to address issues that affect their own health behavior and social welfare. Support patients and exercise positive relationships within professional boundaries as part of a purposeful and caring regime Deal appropriately with request for potentially sensitive information weighing up the need for this information to be provided against patient confidentiality.
