You will work as part of an integrated multi-disciplinary team in the Hospital team to provide high quality social work practice; ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their independence. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
We are looking for individuals who can demonstrate:

Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
An individual who can support and help drive the new Discharge to Assess model.
You must be a qualified social worker and be registered with Social Worker England.

The Hospital team supports all individuals requiring longer term adult social care input on discharge from hospital; working with health professionals to assess their immediate needs, identifying the goals they would like to achieve and the support required to meet these, working with Brokerage to identify suitable providers; ensuring a timely and coordinated discharge from hospital.
This post is a 7 day working post and may be based either in the hospital team or in the community assessing individuals post discharge and you may be required to work between the hours of 8am and 8pm. The successful candidate must be flexible and have a good ability to adapt to change.
