Responsibilities
Conduct thorough assessments of clients' needs and situations.
Develop individualized care and support plans based on assessment results.
Advocate for clients' rights and access to necessary services and resources.
Collaborate with healthcare professionals, community organizations, and governmental agencies.
Provide ongoing support and counseling to clients and their families.
Monitor and evaluate client progress to ensure goals are being met.
Maintain accurate and up-to-date case records and documentation.
Requirements
Master's degree in Social Work (MSW) from an accredited program.
State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
Minimum of 3 years of experience in adult social work or a related field.
Strong understanding of adult mental health and social care legislation.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and as part of a multidisciplinary team.
Demonstrated commitment to social justice and client advocacy.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 16:00
Pay Per Hour: £35.00
